Basic Education Department
We are delighted to welcome both new and returning students to the Basic Education Department of Colegio del Sagrado Corazon de Jesus. Please follow the appropriate steps below for a smooth and guided enrollment process.
Enrollment Steps for NEW Students and Transferees
ADMISSION FORM FOR NEW STUDENTS (PDF)- Apply for BED Entrance Exam / Psychological Test: Secure and accomplish the BED Enrollment Form above and submit it to the Principal’s Office along with the following required documents:
- - Photocopy of Form 138 or Report Card (at least 2nd Quarter; with no academic deficiency and good deportment).
- - Entrance Exam / Psychological Testing Fee - Php 200.00
- Settle Entrance Fee: Pay the Entrance Exam Fee at the Accounting Office (non-refundable and non-transferable).
- Undergo Qualification Process: Present the Official Receipt to the BED Guidance Testing Center. Wait for the test results and proceed to the Principal’s Office for the final interview.
- Settle the Pre-enrollment Fee: Pay a minimum of Php 1,000.00 (downpayment) at the Accounting Office. This amount will be deducted from the total school fees and grants access to your Student Number and Password for all online transactions. Payment can be done onsite or via bank deposit.
- Submit All Admission Requirements: Complete and submit all of the following Admission Requirements to the Principal’s Office:
- - Report Card / Form 138 (original with complete grades)
- - Certification of Good Moral Character (original copy)
- - Certificate of Completion (Grade 10 completer - photocopy)
- - PSA/NSO - issued Birth Certificate (2 clear copies)
- - Baptismal Certificate (1 photocopy)
- - 2x2 Recent ID pictures (2 pieces)
- - ESC Application Form (for incoming Grade 7 only)
- - ESC Certificate (original system-generated) for transferring "ESC grantees" (Grades 8 to 10)
- - SHS Transfer-Out Certificate (system-generated) for Grade-12 transferees who are "SHS Voucher Program recipients"
- Validation of Requirements: Proceed to the Registrar’s Office for document validation and approval.
- Complete Enrollment Process: Visit cscj.edu.ph or the Student Portal to update your online enrollment form and download your System-Generated Admission Form.
- New Student ID Processing: Go to the Audio-Visual Center (AVC) for ID application. Submit your documents, undergo verification, and complete the ID application process. Then, you can pickup and activate your student ID.
Enrollment Steps for OLD Students
ADMISSION FORM FOR OLD STUDENTS (PDF)- Apply for BED Enrollment: Secure and accomplish the BED Registration Form above and submit it to the Principal’s Office along with your Final Report Card or Form-138 (original copy) and School ID.
- Validation of Admission Requirements: Proceed to the Registrar’s Office for validation and approval.
- Settle the Pre-Enrollment Fee: Pay Php 1,000.00 (downpayment; non-refundable and non-transferrable) at the Accounting Office. This amount will be deducted from the total school fees. Payment may be made onsite. Please secure your Student Number and Password, which are required for all online transactions.
- Complete Enrollment Process: Visit cscj.edu.ph or the Student Portal to update your online enrollment and download your Admission Form.
- Student ID Validation: Present your validated Old Student ID and the System-Generated Admission Form to the
School Administration. If valid, a new validation sticker will be issued.
If lost or damaged, secure an affidavit of loss and apply for replacement.
Note: Please secure your affidavit of loss (notarized by a lawyer) to avoid delay.
Higher Education Department (College)
We welcome aspiring students who wish to pursue their college education at Colegio del Sagrado Corazon de Jesus. Below are the steps for new freshmen and transferees.
Enrollment Steps for New College Freshmen and Transferees
ADMISSION FORM FOR NEW COLLEGE STUDENTS (PDF)- Apply for HED Entrance Exam / Psychological Test: Secure and accomplish the HED Enrollment Form. Submit it to the HED Office for verification and preliminary interview along with the following required documents and a Php 170.00 fee for testing:
- - Photocopy of form 138 or Report Card (at least 2nd quarter, with Satisfactory grades in deportment and no academic deficiencies).
- - Entrance Exam / Psychological Testing Fee - Php 170.00
- Settle Entrance Fee: Pay at the Accounting Office (non-refundable and non-transferable).
- Undergo Qualification Process: Present the official receipt to the Guideance Center. Wait for the results of the Psychological Test and proceed to the HED Office or the area Area Chairperson for the final interview.
- Process Enrollment Requirements:if qualified, submit the following documents to the Registrar’s Office:
- - Report Card / Form 138 (original copy with complete grades).
- - PSA / NSO - issued Birth Certificate (1 photocopy).
- - Baptismal Certificate (1 photocopy).
- - Certificate of Good Moral Character (original copy)
- - For Transferees: Transcript of Records and Honorable Dismissal (original copies).
- - Marriage Contract (for married applicant).
- - 2 x 2 ID Pictures (2 pieces).
- Settle Pre-Enrollment Fee: Pay a minimum of Php 1,000.00 (downpayment) at the Accounting Office. (This payment is non-refundable and non-transferable, and will be deducted from the total school fees).
- Complete Enrollment Process: Log in to the Student Portal using your credentials and update your online enrollment form and download your CSCJ System-Generated Admission Form.
- New Student ID - Application and Processing Proceed to the Audio-Visual Center (AVC) for ID Application. Fill out the form, submit the required documents, undergo verification and ID creation. Then, you can receive and activate your new Student ID.
Upon payment, you will be issued your Student's Number and Password for the school's official online transactions.
Payment may be made onsite at the cashier or through bank deposit.
